Ep 68: Post-Bariatric Surgery Life as a Vegetarian

This episode was inspired by a listener question from Kim, who asked about life as a vegetarian after bariatric surgery. I want to start by saying clearly: it is absolutely possible to follow a plant-based or vegetarian diet after bariatric surgery and still reach all of your goals and live a wonderfully healthy life. Here is what you need to know.
The Three Main Challenges
1. Reaching Your Protein Goals
Plant-based protein foods — legumes, tofu, tempeh, edamame, high-protein dairy — are nutritious and wonderful. But they are generally less protein-dense than animal proteins, meaning you need larger volumes to hit the same protein target. This requires intentional planning, especially in the early days when stomach capacity is limited.
2. Managing Carbohydrate Intake
Many plant-based protein sources also contain significant carbohydrates — legumes being the classic example. As you increase your plant protein to meet goals, carbohydrate intake naturally rises too. This is not necessarily a problem, but it requires awareness — particularly in the weight loss phase when we want protein to be clearly prioritised.
3. Achieving Adequate Satiety
Animal proteins are generally denser in protein per gram and more satiating — meaning a small amount keeps you satisfied for longer. Plant proteins can require larger volumes to achieve the same satiety effect, which can be challenging when stomach capacity is restricted.
Strategies to Help
Use unflavoured protein powder to fortify meals — stirred into soups, porridge, or smoothies — without increasing volume significantly. Prioritise the highest-protein plant foods: tofu, tempeh, edamame, lentils, legumes, eggs (if ovo-vegetarian), and high-protein dairy like cottage cheese and Greek yoghurt. And work with a bariatric dietitian who can help you personalise your protein targets and meal balance for your specific type of vegetarian diet. This is not a one-size-fits-all situation.
